Mattawan Consolidated School's Ridership Registration Form for students of all grade levels.

In order for Mattawan Consolidated Schools to provide safe and orderly busing, students are required to register for bus service. All students need to submit a bus registration form this year for each student. Students not registered with transportation will not be assigned to a bus. The bus stop may or may not be located at the home address. However, students will be assigned within the prescribed walking distance. Any changes must be made in writing. PARENTS:  Please feel free to contact the Transportation Department at 269.668.3361 Extension 8710 with questions.  In addition to notifying your child's teacher, please notify the Transportation Department of changes needed to pick up or dropping off your student.  This communication will help assure that your child is delivered safely to the correct location.
