Physical / Consent Form

T-Shirt Order Form

1. Physical / Consent Form:
In order to practice, a physical/consent form must be on file with the office.  IF the form is not complete, an athlete is not able to participate in practice or meets.  Please find a PDF of the form below to print and turn in as soon as possible.

2. Participation Fee:
A fee of $75 is required to participate in cross country if you are a 7th or 8th grader.
If you are a 6th-grade student, the fee is waived.
Payments can be made on Revtrack through the school’s website.

3. Practice:
Regular practice will be run Monday-Friday from 2:50 until 4:15.  Please check the Google calendar for any changes/updates.
Runners should be dressed and meet at the sit-upon by 2:50.  Practice usually begins with attendance and a quick meeting. We might talk about upcoming meets, techniques, or discuss general business/housekeeping.  It’s advised for runners to bring a water bottle to practice and use the time after school when we are meeting at the sit upon to snack! We are usually outside stretching by 3:00 pm.
Please arrange for your child to have a ride by 4:15.  Pick up is behind the school on the blacktop by the tennis and basketball courts.

4. T-Shirt Order:
The shirts will be $15.00, and we hope to place our order ASAP so we can wear them at our first meet! Please pick up and return an order form with payment to the SRC or either coach. Please pay with cash or a check made out to Mattawan Schools.  An attached order form is at the bottom of the page with a picture of the design.

5. Uniform:
Please year our yellow shirts and either black or navy shorts.  On days it is cold, you may wear layers under the shirt and pants if desired. Wear comfortable supportive shoes that you know you can run in and have practiced in.  We don’t want any injuries!
